In order to insert forms into your page you will need a form plugin that can manage the forms. The tested and recommended form plugin to use with our theme is ‘Contact Form 7’

You can create forms with the capabilities of the plugin

I don’t think the form has a pop up button. You could search for other contact form plugins that have such buttons and make tests with them, if this is required.

Try that plugin, or try others. Make tests and choose the one you like. I did not try any of theme, but maybe some other users can make some recommendations if possible.
