That is a contact form, the one from this demo page
– https://demo.tagdiv.com/newspaper_nature/contact/

It is built with the WPForms plugin – https://wordpress.org/plugins/wpforms-lite/
The shortcode provided by the plugin is entered in a column text element. If you do not require it, you can remove it. If you want to use it, install the plugin and enter your own shortcode there.
