Usually wordpress as a CMS is built in as all in one solution, but in our case we are planning to de-couple/split WordPress’s frontend and backend separately. We aim to host the frontend backend separately while they will remain interconnected with wordpress API.
Can we use the same theme? or will the theme require customization?

You need to find this out and let us know because decoupling wordpress is a actually and enterprise and secure method. Even if we plan to move to wordpress VIP then also it will work same way on such context there are change happens on the wordpress structure itself. You need to find out if your theme will work or do we require to custom develop the theme too?

Our theme has been tested only with normal WordPress, was not tested with decoupling WordPress, so most probably it will also not working correctly (maybe it will work, but not 100%).
One of the problems would be if the url from wp site and the one from wp home are different the builder in the theme will not work -> https://i.imgur.com/JQv4USH.png
Also, performing a test for this purpose is not something that can be done in a few minutes, it must be tested over a period of time and see how it behaves and check all options if they are functional. This requires time and resources that we do not currently have. all I can do is put this on our list.
