AMP Issues

I have never quite understood how the AMP is supposed to work or how to set it up. When I have activated mobile theme items and AMPT items my contact form has completely disappeared from my website, naturally not a good thing. Today all of a suddenly I see this show up

Well there hasnt been anything new in a month as Ive been on vacation. I was just back last night and looking things over this morning.
