I’d like to add a 728×90 advert at the very top of every page. There doesn’t seem to be any way to do this other than by adding a custom top bar via API. Is that correct?

Hello,
Anything other than what the theme options can provide will have to be done either with a plugin or custom work.
You can modify the theme files, for example in header style 4, you simply place the header code above the top bar and their position will be switched. It will require a bit of adjusting using CSS though: http://screencast.com/t/dtu6auR4h – http://screencast.com/t/tzqrFloq0t
