Looks like it is there, just loading bottom of page. Might be some table or div wrapping it breaking the layout?

Hmm I’m not seeing it at the bottom of the page. Instead, I’m seeing the “Create your own user feedback survey” at the top of the page. This is the Survey Monkey language that comes at the bottom of the embed.

The problem is caused that you use WP Rockter to minify the js; you can add an ID to the code and try to exclude it from minify it -> https://i.imgur.com/wY5CSVi.png
