How to add Pages Publish Date

I want to add page publishing date just like the post publishing date.
How can I do it?

It will be great If i get “LAST MODIFIED DATE” code too.
I have done POST DATE UPDATE by following this tutorial: techprevue.com/show-last-updated-modified-date-newspaper-theme/

Now, I want to do that for my each PAGE. How can apply similar process for PAGES too?
