I am not using AdSense and simply want to place a 728 x 90 image banner in the header with an href link to my contact page. I cannot find out how that is done.
In the future, I may also wish to place similar images in sidebars. Can you please tell me if this is possible? If so, please provide instructions.

The situation is not resolved, unfortunately. On both Firefox and Chrome browsers, the banner does not display at all. On IE, it displays partially (bottom part is cut off). I tried the custom css that you recommended and it had no effect on any of these browsers.
Your banner appears in Google chrome without any issue. Might be you have installed ad blocker in your browser or it might be internet security softwares like kaspersky?
Why don’t you rename the banner file from “header-ad.gif” to “headerb.gif” for testing purpose?
Yes, I have Adblock Plus installed on both browsers. I never even thought about that. I have since renamed the image files and they seem to work. I also have added the tablet and mobile images that I made; unfortunately, I don’t own a tablet or smart phone, so can’t test them.
