How to update the theme without losing custom modifications?

How can I update the theme without losing all the modifications I have made? I try to use the auto-updater, but when I click on update it does nothing.

You should make a full backup to your database if you have some custom modifications to avoid losing all of your data.

After the update has been made succeed you should load your backup.

So how do I backup the modifications to the theme, but then apply them again when the update is available? I don’t really understand.
