Image in posts not displaying correctly – too large

The image shown above is 131px x 11px. I included in a post as follows:

However, when I look at the HTML on the post displayed (above), it reads:

How can I correct this. (Many posts are affected.)

It was a plugin causing the problem – Imagify – image optimizer.
