Error creating forms using the NINJA FORMS plugin..

I want to create surveys on my page, ask questions to my visitors, take feedback from them.
Is there a particular way that can be done ?
i did a little search and found this free plugin NINJA FORMS , which helps us create forms using text fields , radio buttons etc and also record the submissions of the users saving results in the database and is displayed simply.
but when i try saving the form , i get this error.

Forbidden
You don’t have permission to access /wp/wp-admin/admin.php on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Hi,

You should check your file permissions. You can find more info about this here: http://codex.wordpress.org/Changing_File_Permissions
Please also note that we haven’t made any tests with this plugin so it would not work so make sure you also check this without any plugins active. If is still not working we suggest you find another for this.
