Smart list showing 2 images for first item

This happened on this article – so I had to remove the first image and use it as the featured image (kind of). I don’t know how to get rid of that duplicate image. Any help would be appreciated.

And now it’s not duplicating it…. I inserted an image for the first smart list item and now it’s only showing one… not sure why or how. If it comes up again I’ll let you know. Thank you!

In the preview it only showed one image, published it’s 2.
