social share and like buttons on posts list

Is there any option or way to show on social share and like buttons on posts lists?
Lets say,I make a page from 20 posts. Each post section has own social icons ( shows how many people shares from facebook and twitter ) . And if you click one share button, you share the post not the page you listed.

this is an example page that i create with photoshop, to make it easy to understand what i would like to do.
