This can be if you are using small images.
Also another case can be from a compress plugin that helps to optimize the website. Please make a test using only the theme plugins and see if the problem is still persisting.

my problem was solved by downloading the “generate thumbnails” plugin. once i installed it and activated the plugin, all the images were sharp and in focus.
