I think you can do that using the built-in ad manager, using the ‘insert after n paragraph’ option ?
I have not seen this. Where is this in the topic? Can you show the screenshot?
That would mean the theme has not been updated or installed properly. You could try a theme re-install by FTP

I’ve always updated the topic by FTP and very neat.
I will not reinstall the topic, because I do not want to risk completely killing the site.
That would solve the issue based on user feedback, as it can be seen in the topics mentioned above. We cannot recreate this issue on our end. You could make a full backup before to be safe, and simply reinstall the theme (be sure to completely remove old theme and upload new one, not overwrite).
