Hello,
You can create a page and set the code for the Mailchimp (you can use the column text element and set the code in the text area). After you create the page you can set it to o modal popup -> https://forum.tagdiv.com/modal-popup/

You need to create the form in Mailchimp and after setting the code between head tags, this can be set in our theme in Theme Panel -> Analytics/Js Codes https://i.imgur.com/3cet4cy.jpeg
