The image at the top of your post is the featured image which you can set from here http://screencast.com/t/8ymScHbi. The first thing we recommend in these types of situations is to use post template style 5 as shown in the documentation https://forum.tagdiv.com/smart-lists/ but you could also use your preferred post style type but with a different featured image than those in your smart list.

I’m getting the same issue. The screencast doesn’t really help. I’ve created a featured image, but I’m still getting multiple images for smart lists.
When you set a featured image, it will be automatically be placed at the top of your post. This is an empty post, just with a featured image set. – http://screencast.com/t/OCwsucVlBQZd
Please use a different featured image than your list and it should not display twice.

There are a couple of things to test if the image appears twice on smartilsts. The first one is to have the latest version of the theme. If you do not, please update your theme.
The second is to disable the photon feature from jetpack.
If the problem is still there, there is another known issue when adding special characters in the alt text of the image. Please make sure you use only standard characters.
