Thanks for all your helps.
Its actually a problem with W3 Total Cache. Its cached files are affected by PUA.Phishing.Bank virus.
No support for this plugin since last 5 months. Trying to sort out the issue.

Hi Andrei,
I have upgraded both wordpress and Newsmag. Deleted all plugins and rechecked.
Only one case where I did it successfully – Manually placing Sync google ad code in single.php
Check any random post.
But the default ADs blocks in Theme panel are not taking any code. So I am not able to place ad in home page/header/side bar. 🙁
You have rightly identified, Ad codes are changing in my site.
I am still doubting on W3 Total Cache Javascript minification.
Any suggestion please ?

I think the root casue may be W3 Total Cache Javascript minification.
I have disabled minify option, even disabled and delete the plug in.
But things have not changed. How to restore ?

No message or policy violation in my Adsense. Here is the code –

An interesting observation – The header ad is showing properly in all Error 404 pages!

Checked as you said. Placed a simple text box in default side bar.
The same error massage appear twice.

Deactivated all plugins. Same error shown.
Installed a third party Adsense plugin. Problems persists.
