At the moment, the theme does not offer any support for Disqus in AMP plugin, sorry! We have added to our list of improvements and maybe, this can be considered to be a new feature in our updates.

If you want to add such a form in our theme, you can use the Contact Form 7 plugin for that because this is tested plugin and seems to return well results. That plugin generates some shortcodes with the Forms and then, these can be added in a Column Text or Text with Title element to display on the Frontend.
