I checked the reported “bug” and indeed this is how it is acting. If you are using a banner in sidebar section and you set it in a place where the width is bigger, then the sidebar add will take the full width of that row.

What I forgot to mentioned is when you set a placeholder, set it in full image size, and not a thumb, also the steps you did with “thumbnail regeneration” are ok if you changed the thumbs dimension you used.
Now according to your screenshot it is like some images are no loading, most of the time when this problem appear to our users was related to a cache plugin that has been used, maybe now was the same.
