I think that your problem is regarding theme saving, right? 1)Yes, the AMP will work for all the pages but only if you will use the Mobile Theme. Otherwise, only the single pages will be discoverable by the AMP (simple pages which are not created with page builder).

After you make the update please check if theme plugins are active, usually tagDiv Composer will activate automatic but it needs to be checked, also if you are using cloud template update the tagDiv cloud library and activate it.

This is not a simple task and it involves doing such additional customizations through the code and we cannot provide this at the moment, sorry! As a hint, you can try to check the code from the single template.php or loop template, where this form is called and try to edit and place it as you wish. Another alternative, in this case, would be to find a plugin with this functionality and check it with the theme and see how it works.
