I tested this feature on my end but could see no issue.
This could be a plugin conflict so please disable all plugins except visual composer, clear cache and reset cdn files if you use it. Please also check any custom code you might have added to the theme panel or the theme files. If you use a child theme, please switch to the main theme to make sure it does not affect the proper functionality of the theme.
2. There is no simple way of adding an image inside the block titles, sorry. You cna add a font awesome icon, but we have not tested an image inside that field:

You can add this code in the theme panel-> custom css box to remove some of the margins and padding that create the white space:
.td_block_related_posts{
padding-bottom: 0!important;
}
.single .comments{
padding-top: 0px!important;

}

If you really need to use it like that, then you can overwrite it using custom css in Theme Panel > Custom Css – http://screencast.com/t/bYOalphRQ

I guess you use php 7 so please try to modify the following parameter – http://prntscr.com/dj933m and see how it goes. The current theme version was not tested and adjusted for php 7.
When block offset is not set, it defaults to empty string. On previous versions of php it was automatically converted to integer 0.

It is strange…please try to disable all the active plugins, leave just Visual Composer to active, clear all your caches, purge CDN files and install only the plugins that come bundled with the theme packet and rule out any conflicting plugins.

Please keep in mind that you can disable the blocks/modules which you do not use in our theme from Theme panel, like this -> http://screencast.com/t/3nzgXRxBbr If you will turn off these settings, the theme will not load the images corresponding to these unused modules.

1)Our theme does not have any such an option that allows you a way to add there your desired ad, sorry! If you want to do this, you should edit the theme core file and add the do_shortcode method for this. For example, I have noticed that you are using the single post template style 1.
