Sorry for this inconvenience but I guess that issue happens due to your untested plugins which might interact with the database and remove the theme activation status. Please try to disable all of them, leave just only required plugins to be active, clear all the caches and check the results.

Please notice that the first step in WordPress debugging is to deactivate all of the untested plugins. I have checked your list above and I saw that you have some untested plugins, sorry! Please remove all of these, revert the theme to the default state, clear all of the caches and check the results. These plugins might interact with the theme and cause some errors.
Please keep in mind that error does not appear on a fresh install.
