Make a test and deactivate ALL plugins except tagDiv ones, also if you are using a cache plugin, deactivate it, clear all cache and try again.

2. I can see that your subscription was made with a bank transfer, and there is an “issue”. For the bank payment, the user should make a bank transfer, and after receiving the money, the admin should activate the subscription manually from Opt-in builder -> Subscriptions (edit).
